What To Bring

All patients should bring the following:

• Any films, CTs, bone scans, PSA labs, or any pertinent medical information
• Current insurance cards
• If you have an HMO or other insurance requiring referral authorization, you must bring a copy of the authorization with you; we will gladly see you without your authorization, however, you will be required to pay for your visit at the time of service.
• A list of medications you are currently taking
• Photo ID
• Please print and complete all of the forms listed under Forms below.
